About The Position

This position is eligible for a pay increase upon successfully completing the 3-month orientation period. The Floating Case Planner is responsible for promoting the safety, permanency, and well-being of children placed in foster care through Seamen’s Society by providing case coverage and support as needed. This role requires flexibility in managing a frequently changing caseload across the Staten Island and Brooklyn locations. Under the direction of program leadership, the Floating Case Planner provides immediate case coverage, ensuring timely completion of critical tasks, service planning activities, documentation, and follow-up responsibilities. Working collaboratively with children, families, foster parents, and service providers, the Floating Case Planner serves as the primary coordinator of service planning efforts and leads the development, implementation, and monitoring of individualized case plans.

Requirements

  • Proficiency in Microsoft 365 applications and working knowledge of CONNECTIONS.
  • Strong interpersonal and teamwork skills, with excellent planning, organizational, time-management, and follow-up abilities.
  • Ability to perform the essential functions of the position with or without reasonable accommodation.
  • Demonstrated ability to analyze, troubleshoot, and resolve recurring case-related challenges in a timely and effective manner.
  • Bachelor’s degree in Social Work or a related field required.
  • A minimum of two years of case planning experience required.
  • Ability to travel between the agency’s Staten Island and Brooklyn locations as needed.
  • Flexibility to work varied hours based on program, client, and operational needs.

Responsibilities

  • Provide case coverage and support for assigned cases within the Enhanced Family Foster Care (EFFC) program as directed by program leadership.
  • Review assigned cases immediately upon notification from program leadership to assess case status, identify critical needs, and initiate necessary case planning activities.
  • Establish timely contact with children, families, foster parents, and service providers to ensure continuity of services and compliance with required casework contacts.
  • Manage and complete time-sensitive tasks, follow-up activities, and service coordination responsibilities to maintain effective case oversight.
  • Engage families in service planning and begin required contacts and interventions promptly upon assignment.
  • Monitor child safety, well-being, and permanency through regular casework contacts, home visits, and ongoing assessment of family functioning.
  • Partner with families to develop, implement, and monitor service plans that address identified strengths, needs, and permanency goals.
  • Collaborate with interdisciplinary team members and community providers to support each child's medical, mental health, educational, housing, and vocational needs, as applicable.
  • Maintain accurate and timely documentation of all case activities in CONNECTIONS in accordance with agency and regulatory requirements.
  • Participate in Family Team Conferences (FTCs), Family Assessment and Service Plan (FASP) meetings, and other service-planning activities within established timelines.
  • Attend Family Court proceedings for assigned cases and provide written reports and verbal testimony regarding family progress and service-plan outcomes.
  • Prepare and submit court reports, permanency hearing reports, and other required documentation in accordance with agency standards and deadlines.
  • Support and facilitate meaningful, consistent family visitation and communication to strengthen family relationships and advance permanency goals.
  • Identify and address barriers to reunification, kinship placement, adoption, or other permanency outcomes through assessment, advocacy, and service coordination.
  • Participate in treatment planning, Trauma Systems Therapy (TST) meetings, and other multidisciplinary forums to support child and family well-being.
