Requirements

• Current Massachusetts Nursing license in good standing.
• 2+ years of nursing experience required.
• Strong clinical assessment skills and the ability to prioritize and triage patients.
• Exceptional communication skills and the ability to work within a team structure.
• Excellent organizational skills and time management skills.
• Ability to work independently and understanding of how to access resources for patients.
• Strong computer skills.

Nice To Haves

• Registered Nurse (bachelor’s degree preferred).

Responsibilities

• Providing direct patient care in the patient’s home.
• Conducting assessments.
• Developing care plans.
• Working collaboratively with the patient’s physician to provide optimum care in the home.
• Coordination of care.
• Identifying the need for other services in the home.
• Travel/float throughout the following areas: Hanover, Norwell, Marshfield, Rockland, Abington, Whitman, East Bridgewater, West Bridgewater and Bridgewater.
