About The Position

The Float Pool Nurse is responsible and accountable to a group of patients for a designated time frame and provides care to the patients via therapeutic use of the nursing process, in collaboration and integration with other health care team members. S/he demonstrates the knowledge and skills necessary to provide care relevant to the patient(s) under his/her care, applying principles of growth and development over the life span. S/he assesses and interprets patient data needed to identify each patient's requirements relative to his/her age-specific needs. S/he functions in the role of a primary RN in at least three nursing areas including Medical-Surgical and one Critical Care area.

Requirements

  • Current New Mexico or Multi-State Licensure
  • BLS (AHA)
  • ACLS (AHA) within 3 months of hire
  • Certifications required for Nursing Specific areas within 6 months of cross training: SCU, ED, Med-Surgical, House Supervisor, MATCH, PPACU, Infusion, Cancer Center and BHU.
  • Minimum two (2) years’ experience in an acute care setting required

Nice To Haves

  • Two (2) years Medical Surgical experience preferred
  • One (1) year critical care experience preferred

Responsibilities

  • Collect health data, determining priority, using appropriate assessment techniques, involving the patient, significant others, health care providers, and other GRMC personnel, systematically and ongoing, and documenting relevant data in a retrievable form.
  • Analyze assessment data in determining diagnoses, deriving plans of care from assessment data, validating plans with the patient, significant others, health care providers, and records, and documenting plans to facilitate expected outcomes and care plan.
  • Identify expected outcomes individualized to the patient, derived from diagnoses, documented as measurable goals, mutually formulated with the patient and health care providers, realistic, attainable, including a time estimate for attainment, and providing direction for continuity of care.
  • Develop a care plan that prescribes interventions to attain expected outcomes, individualized to the patient's condition or needs, developed with the patient, significant others, health care providers, and other GRMC personnel, reflecting current nursing practice, documented, and providing for continuity of care and discharge follow-up.
  • Implement the interventions identified in the care plan, ensuring interventions are consistent with the established care plan, implemented in a safe, appropriate, and timely manner, and documented.
  • Evaluate the patient's progress toward attainment of outcomes, systematically and ongoing, documenting the patient's responses to interventions, evaluating the effectiveness of interventions in relation to outcomes, using ongoing assessment data to revise diagnoses, outcomes, and the care plan as needed, documenting revisions, and involving the patient, significant others, health care providers, and other GRMC personnel in the evaluation process.
  • Systematically evaluate the quality and effectiveness of nursing practice, participating in quality-of-care activities such as identifying aspects of care, indicators, collecting data, analyzing data, formulating recommendations, implementing activities, participating in interdisciplinary teams, and developing policies and procedures to improve care.
  • Evaluate his/her own nursing practice in relation to professional practice standards and relevant statutes and regulations, regularly engaging in performance appraisal, identifying areas of strength and development, seeking constructive feedback, taking action to achieve goals, and participating in peer review.
  • Acquire and maintain current knowledge in nursing practice, participating in ongoing educational activities related to clinical knowledge and professional issues, seeking experiences to maintain clinical skills, and seeking knowledge and skills appropriate to the practice setting.
  • Contribute to the professional development of peers, colleagues, and others, sharing knowledge, skills, and information, providing constructive feedback, and contributing to an environment conducive to the clinical education of nursing students.
  • Ensure decisions and actions on behalf of patients are determined in an ethical manner, guided by The Code for Registered Nurses and related ANA position statements, maintaining patient confidentiality, acting as a patient advocate, delivering care in a non-judgmental and non-discriminatory manner, preserving patient autonomy, dignity, and rights, and seeking available resources to help formulate ethical decisions.
  • Collaborate with the patient, significant others, criminal justice system personnel, and health care providers in providing patient care, communicating with stakeholders, consulting with health care providers, and making referrals.
  • Consider factors related to safety, effectiveness, and cost in planning and delivering patient care, evaluating factors when two or more practice options would result in the same expected patient outcome, assigning tasks or delegating care based on patient needs and provider knowledge/skill, and assisting the patient and significant others in identifying and securing appropriate, available services.
