The Float Pool Licensed Practical Nurse (LPN) provides essential basic nursing care, including assisting with resident assessments upon admission and on an ongoing basis. This role requires the LPN to stay informed about residents' conditions and communicate any significant changes to the interdisciplinary team (IDT), accurately documenting all assessments and seeking guidance when needed. Responsibilities extend to answering call lights, assisting with residents' Activities of Daily Living (ADLs), and actively participating in the care planning process, including updating care plans as interventions dictate and communicating these plans to the IDT, residents, and their families. The LPN implements nursing and physician orders, addresses residents'/families' educational and environmental needs, and ensures safe and timely medication administration and treatments, including IV fluids. Critical thinking is crucial for managing changes in residents' conditions, requiring timely physician notification using the SBARR communication tool. The LPN must maintain professional composure during crisis situations, monitor care delivery effectiveness, identify problem areas, and utilize the QAPI process for resolution. Continuous awareness of residents' status is necessary to quickly identify behavioral changes or other untoward signs. The role mandates a minimum of twelve hours of nursing-related continuing education annually and a commitment to staying current with best nursing practices and Bethesda's policies. This position requires flexibility to work at all Bethesda Health Group locations, including Rehab, Long-term Care, Memory Care, and Assisted Living settings, potentially at different locations daily. Effective communication with residents, families, and all organizational levels, along with maintaining confidentiality, is essential. Other duties may be assigned as appropriate.
