In this RN Field Service Coordinator, Nursing Facility role, will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs. Our team is made up of RNs, LVN’s, and CACs dedicated to helping members achieve their health care goals. We work closely with the nursing facilities in which our members reside. If you are in or within a 60 mile radius driving distance from Seguin, Converse, Selma, Brook City Base, Floresville and LaVernia to support the Bexar County, TX Service Delivery Area (SDA), you will have the flexibility to work remotely as you take on some tough challenges. Travel required in local areas about 75%25 of the time based on business needs. This is a field based position with a home based office.

Requirements

  • Current, unrestricted RN license in the state of Texas
  • 2+ years of experience working within the community health setting or in a health care role
  • Intermediate level of computer proficiency (including Microsoft Outlook, Teams) and ability to use multiple web applications
  • Valid driver’s license, reliable transportation and the ability to travel up to 75%25 in this ‘assigned region’ to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' offices
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
  • Reside in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
  • Reside in or within a 60 mile commutable driving distance from Seguin, Converse, Selma, Brook City Base, Floresville or LaVernia

Nice To Haves

  • 1+ years of experience with long term care services and support, Medicaid or Medicare
  • Knowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulations.
  • Demonstrated ability to create, edit, save and send documents, spreadsheets and emails
  • Reside within one of the following areas: Seguin, Converse, Selma, Brook City Base, Floresville or LaVernia, TX
  • All employees working remotely will be required to adhere to UnitedHealth Group’s Telecommuter Policy

Responsibilities

  • Assess, plan, and implement care strategies that are individualized by member and directed toward the most appropriate, lease restrictive level of care
  • Identify and initiate referrals for member transition to community request and provide assist with social service programs, including financial, psychosocial, community and state supportive services
  • Manage the care plan throughout the continuum of care as a single point of contact
  • Communicate with all stakeholders for the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
  • Advocate for members and families as needed to ensure the member’s needs and choices are fully represented and supported by the health care team Visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices
