About The Position

The RN, Nights and Weekends provides after-hours field visits from 8 am to 8 pm to patients, families, and community partners. This role establishes and maintains positive relationships with team members, patients/families, and referral resources. The position involves completing clinical admissions, explaining services, identifying admission barriers, educating patients/families, completing consents and initial assessments, providing information to physicians, initiating care plans, and performing skilled nursing interventions. The role also involves acting as a company representative at assigned facilities, communicating with admissions, and providing care for GIP patients. Responsibilities include making after-hours visits, providing compassionate customer service, assessing patient/family needs, managing direct care, demonstrating clinical skills in case management and symptom management, collaborating with other professionals, educating patients and families, reporting changes in patient condition, attending patient deaths, and completing documentation. The role may also require reporting to designated locations during emergencies to ensure continuity of services.

Requirements

  • Current license as RN in the state where the employee will be working
  • Minimum of one (1) year nursing experience

  • Active BLS for healthcare professionals from the American Heart Association or Red Cross.

  • Provides compassionate customer service to patients and families.
  • Assesses patient/family and identifies immediate needs and appropriate nursing interventions by phone and/or during visit.
  • Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical and biological components, and appropriate nursing intervention and follow-up.
  • Demonstrates clinical skills regarding case management, application of company policy and procedure, physical assessment, intravenous therapies, pain medication titration, symptom management, and patient/family relationships.
  • Prioritizes, consults, refers, and collaborates with other professionals to promote quality and continuity of patient/family care.
  • Educates patient, family, caregivers and other health professionals about disease process and decline, palliative interventions, care giving, dying process, and safety practices.
  • Reports changes in the patient’s condition to appropriate members of the IDG or other health professionals.
  • Attends patient deaths; prepares body for transport; oversees disposal of medications; and provides bereavement support.
  • Completes documentation per policy and procedure.
  • During times of emergencies (i.e. Hurricanes, etc.), the RN, Nights and Weekends may be required to report to work at a location designated by the company, to ensure continuity of services.
