About The Position

Advantmed is seeking a licensed, dependable Nurse Practitioner to conduct structured, in-home wellness and risk-adjustment assessments for diverse patient populations with varying healthcare needs. This PRN role offers flexibility with consistent visit availability and strong operational support. It is best suited for NPs who value autonomy, professionalism, and accountability and are looking for steady supplemental income, not short-term or ad-hoc work. This role offers paid training and standardized onboarding, with compensation of approximately $100 per completed visit plus mileage. Most providers complete 4–5 visits per day, based on availability, with visits ready to schedule immediately. You can choose your own weekday availability and receive dedicated coordinators, scheduling, and tech support. As a Field Nurse Practitioner, you will conduct in-home Annual Wellness Visits and chronic condition assessments. This includes performing focused histories, exams, vitals, medication reviews, and screenings such as Hemoglobin A1C, Spirometry, KED (Kidney Health Evaluation), DRE (Diabetic Retinal Exam), and FIT. You will also educate members on preventive care and chronic disease management and document visits in the EMR to support value-based care and risk adjustment. All assessments are standardized, with training, equipment guidance, and EMR support provided.

Requirements

Active, unencumbered NP license in the applicable state
3+ years of patient care experience preferred
Comfortable with EMR systems and independent work
In-home or risk-adjustment experience preferred
Willing to obtain additional licensure if needed (supported by Advantmed)
Daily travel within a 55-mile radius of the assigned county
Must have a valid unencumbered NP License for the state you will be working in
This role requires travel up to a maximum 55-mile radius originating in the assigned job posting county
Preference is given to weekday schedules
Preference is given to standard working hour schedules
Previous in-home risk assessment experience preferred
Previous 1099 (PRN) experience is preferred but not required
3 years patient care experience preferred (primary care/adult/geriatric, EMR)
May be requested to obtain additional NP licensure supported by Advantmed
Access to reliable transportation that will enable you to travel to members’ homes within a designated area
Strong ability to work within our EMR system
Ability to work independently
