About The Position

Paradigm is seeking a full-time (0.75 - 1.0 FTE) Field Nurse Case Manager to cover the Houston, TX. This is a salaried position: salary range is $70,000 - $98,000 annually (based on FTE). A Field Nurse Case Manager is a hybrid position. The candidate must be located in the Houston, TX, area due to regular local travel (80% of the time) for in-person patient appointments. Candidate must be willing to travel and be licensed in Texas. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers. Candidate must be willing to learn and take catastrophic cases.

Requirements

  • Current, unencompacted Registered Nurse (RN) license in Texas
  • Skills required for success: organization and timeline adherence skills, PC and technology skills, communication skills (written, verbal, and interpersonal)
  • Ability to operate autonomously with minimal oversight
  • Skilled at patient education
  • Valid Driver’s License, and good driving history
  • Must be located in the Houston, TX, area
  • Willing to travel (80% of the time for in-person patient appointments)
  • Licensed in Texas

Nice To Haves

  • Experience in case management of workers comp cases
  • Recommended certifications (CCM, CRRN, CDMS, CRC) preferred, REQUIRED within two years of the date of hire.
  • Bilingual Spanish

Responsibilities
