Field Nurse Care Manager & Patient Advocate (RN)

Unicity Care Management•Paramus, NJ

About The Position

Unicity Care Management supports individuals and families through important healthcare decisions, including new diagnoses, complicated hospital discharges, and chronic conditions. The Nurse Care Manager and Patient Advocate provides high-touch, concierge-level nursing support and serves as the primary contact for clients and their families. The role carries a deliberately small, private-pay caseload so the nurse can become familiar with each client’s history, family dynamics, and needs. It is care management rather than bedside nursing, with independent clinical judgment and a schedule built around client visits. This is a field-based, hybrid role requiring regular travel and is not an in-office position. The nurse conducts in-home assessments, develops and updates care plans, coordinates providers and resources, manages transitions between care settings, advocates for clients and families, and responds to urgent needs that may arise outside standard business hours. The nurse may accompany clients to medical appointments and other care-related visits. Compensation is a $100,000–$115,000 base salary, plus a monthly performance bonus based on billable hours. The posting states first-year total earnings of $106,000 to $130,000+. The company provides a dedicated company phone number and a laptop, and reimburses mileage and tolls. The role includes health, dental, and vision coverage; a safe harbor 401(k) with company contributions fully vested from day one; 15+ PTO days and paid holidays; and voluntary life insurance and disability coverage.
