About The Position
Lightways Hospice and Serious Illness Care, founded in 1982, is an independent, nonprofit healthcare provider serving 11 counties in northwest Illinois. They offer serious illness care, hospice care, and grief support for all ages. Most patient care is delivered in the field, with team members providing services in homes, nursing facilities, or assisted living facilities. They also operate a 24-bed inpatient unit in Joliet, and non-clinical staff are based at the Joliet administrative offices.
Requirements
- High school diploma or GED
- Meets the training requirements of Illinois
- Current CNA certification
- Requires a valid driver’s license and a dependable, properly insured vehicle that complies with state and organizational standards
Responsibilities
- Provide compassionate care, emotional support, and companionship to patients and families
- Assist with personal care, mobility, and daily living activities
- Monitor, document, and report patient condition and changes to the RN
- Ensure patient safety and proper use of equipment
- Maintain a clean, safe, and comfortable home environment
- Follow care plans, policies, and participate in training and team activities
