About The Position

The field CNA is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The field CNA is responsible for observing the patient, reporting these observations and documenting observations and care performed.

Requirements

  • High school diploma or GED.
  • Meets the training requirements of Illinois.
  • Current CNA certification.
  • Requires a valid driver’s license and a dependable, properly insured vehicle that complies with state and organizational standards.

Responsibilities

  • Provide compassionate care, emotional support, and companionship to patients and families
  • Assist with personal care, mobility, and daily living activities
  • Monitor, document, and report patient condition and changes to the RN
  • Ensure patient safety and proper use of equipment
  • Maintain a clean, safe, and comfortable home environment
  • Follow care plans, policies, and participate in training and team activities
  • Position requires outside travel
