About The Position

This posting is intended to fill an existing vacancy for our Field Case Manager role. The Field Case Manager serves as the primary contact for physicians, communicating with patients and program stakeholders to build and maintain trusted relationships in their designated area. They will educate healthcare providers and their staff on program services, patient support resources, and assist in navigating therapy, reimbursement, and enrollment processes. The role involves reviewing patient charts, ensuring completion of all required documentation, and taking ownership of the special authorization process for timely and accurate submissions. Ongoing communication and updates to physicians regarding patient status and program changes are essential. The Field Case Manager will adapt program elements to meet unique provider needs, personalize support for both in-clinic and remote care, and proactively identify areas for quality improvement. Other duties as assigned.

Requirements

  • Registered Nurse (RN) or related degree (BSc in Pharmacy or other Science degrees) or equivalent transferable skills.
  • Knowledge of public or private third-party reimbursement arena.
  • Ability to communicate effectively both orally and in writing and in both English and French (oral and written) for the Quebec and New Brunswick Provinces
  • Exceptional customer service and interpersonal skills
  • Ability to manage multiple tasks and priorities at once
  • Comprehension of medical terminology
  • Strong analytical and organizational skills
  • Ability to engage with a physician and his/her staff as a trusted and well-informed medical counterpart
  • Excellent time management skills & sense of urgency
  • Ability to collaborate effectively with program associates
  • Ability to work autonomously in a remote setting
  • Detail and results oriented
  • Ability to adapt to change and ambiguity
  • Strong computer skills and knowledge of Microsoft Office
  • Hold and maintain a valid, non-expired, unrestricted driver’s license and possess a car
  • Adhere to Performance Standards and Procedures required by the program
  • Must have a private, dedicated home office space that accommodates equipment supplied by Innomar Strategies

  • Bilingualism is an asset (requirement for province of Quebec).
  • Previous Case Manager experience is an asset.

Responsibilities

  • Serves as the primary contact for physicians.
