About The Position

The Family Team Conference Coordinator plans, prepares, and facilitates Family Team Conferences. This role assists in ensuring that team and family meet the requirements of the support plan and participates in the organization’s performance and quality improvement efforts. The coordinator collaborates with a multidisciplinary team to identify family strengths, supports goal completion, and coordinates interdisciplinary/interagency communication. They also act as a resource to Family Support Coordinators and other staff, maintain fidelity to the SEEDS Model, and adhere to virtual office procedures.

Requirements

  • Bachelor’s Degree in Social Work with two (2) years experience within the Early Childhood, Family Systems and/or Educational systems; or Associates Degree with three (3) years experience within the Early Childhood, Family Systems and/or Educational systems; or High School Diploma with five (5) years experience within the Early Childhood, Family Systems and/or Educational systems.
  • Must possess and maintain a valid Florida driver’s license with no record of criminal driving offense of license suspension.
  • Must possess means of providing job related transportation and show proof of required insurance.
  • Must be insurable under CHN’s current auto insurance policy.
  • Must be able to work flexible hours, including early morning, evenings and weekends.
  • Must successfully complete and maintain FDLE clearance, Federal background and state criminal background check and sexual predator screening.
  • Ability to communicate effectively, verbally and in writing.
  • Ability to establish effective working relationships with families, coworkers, supervisors, and other individuals.
  • Possess effective interpersonal skills and conflict resolution skills.
  • Knowledge of agency’s organizational structure, standard operating procedures, and policies.
  • Knowledge of child abuse or neglect reporting procedures and methods.
  • Knowledge of child development inclusive of developmentally appropriate strategies.
  • Knowledge of intervention and behavior management methods, strategies, and techniques.
  • Knowledge of quality documentation as required by agency standards, rules, and regulations.
  • Ability to review family plans, and to provide follow-up services.
  • Must be able to interact in a professional manner with internal and external customers.
  • Ability to work as a multi-disciplinary team member in a positive productive manner.

  • Experience and training in team conferencing preferred.

Responsibilities

  • Plans, prepares, and facilitates Family Team Conferences for families of SEEDS.
  • Actively participates in a multidisciplinary team to discuss family progress, identified needs, support plans and methods of increasing effectiveness of the group's teamwork.
  • Completes timely documentation in accordance with program requirements.
  • Ensures family and interdisciplinary team is well informed and included in support planning.
  • Schedules and leads family conferencing team meetings.
  • Collaborates with multidisciplinary team to identify strengths of the team to support the completion of the family’s goals.
  • Coordinates interdisciplinary/interagency communication and activities. Intra-agency coordination provided as needed.
  • Maintains fidelity to the SEEDS Model.
  • Actively participates in quality improvement measures to ensure that contract outcomes are being met.
  • Acts as resource to Family Support Coordinators, collaborative partners providing services to the family, department and other CHN staff and families.
  • Attends training, participates in internal/external committees, and attends all internal meetings as directed.
  • Works within the philosophy and function of Children’s Home Network.
  • Adheres to virtual office procedures.
  • Performs other duties as assigned.
