Family Support Service Navigator (FT)
About The Position
The Family Support Service Navigator role is a full-time position focused on supporting families who have a family member with an Intellectual Disability. Responsibilities include case management, assisting families in accessing generic services, assessing needs, developing Family Support Plans, and providing information and referrals. The role requires adherence to agency standards and DDS regulations, close collaboration with DDS Service Coordinators, maintenance of accurate financial records, and outreach to generic services and other state agencies. Ensuring timely completion of all documentation is also a key duty.
Requirements
- Valid driver's license
- Dependable vehicle
- Experience working with individuals with intellectual disabilities
