POSITION SUMMARY: The Family and Individual Support and Education Liaison promotes community awareness, inclusion, and participation for families of children and youth who need additional support, as well as adults with developmental disabilities. This role coordinates multidisciplinary supports, delivers home- and community-based services, works within the school system and other formal support systems as required. The role partners with families to support child development and inclusion. The position requires a strong understanding of child development, disability, family systems, and community resources, along with a strengths-based and culturally responsive approach.

Requirements

  • A 4-year degree from an Institution of recognized standing in a field related to Family Support (e.g. education, social work or psychology).
  • Maintain a satisfactory criminal record check throughout employment.
  • A valid BC Class 5 Driver’s License and maintenance of a satisfactory driving record.
  • Required to use personal vehicle.
  • First Aid/CPR & MANDT training.
  • Requires union membership.

Responsibilities

  • Coordinate multidisciplinary supports.
  • Deliver home- and community-based services.
  • Work within the school system and other formal support systems as required.
  • Partner with families to support child development and inclusion.
  • Facilitate a process of individual planning and service coordination for families of children with additional support needs.
  • Work proactively and collaboratively with community agencies and government systems and services, including related service providers.
  • Work with community agencies and government systems and services, including the education system, Ministry of Children and Family Development, and Community Living British Columbia, Ministry of Health, Ministry of Social Development and Poverty Reduction, and others as required.
  • Support families and individuals in addressing core issues related to community living across an individual’s lifetime within a family and person-centered/directed model.
  • Build relationships that strengthen inclusion and participation within the community.
  • Support children, youth and adults to develop self-advocacy skills, self-determination and choice.
  • Develop and facilitate new programs and initiatives to advance the well-being of children, youth and families in alignment with the organization's overall plans and objectives.
  • Represent and advocate for children and youth with disabilities, their families and the organization on various local and provincial committees.
  • Develop and facilitate a range of regular education, support and networking events for families.
  • Develop and facilitate a range of social and educational group opportunities for youth to advance their self-determination, choice, independence and community connections.
