About The Position

Housing Hope and HopeWorks agencies are the largest non-profit developer of affordable housing and provider of human services in Snohomish County and Camano Island. Our mission is to promote and provide affordable housing and tailored services to reduce homelessness and poverty for residents of Snohomish County and Camano Island. We achieve this through a variety of interventions including wrap around services, tailored case management, children’s services, subsidized housing opportunities, adult life skills classes, and access to employment, education, and on-the-job training support. The Family Support Coach provides trauma-informed assessment, case management, and support services to individuals and families transitioning out of homelessness. This role partners with residents to strengthen housing stability, increase income, and build the skills needed for long-term self-sufficiency. The position also contributes to the ongoing development and improvement of service delivery models to better meet participant needs.

Requirements

  • Bachelor's or associate degree in social work or related field.
  • 3+ years’ professional work (or other relevant) experience
  • Demonstrated work experience in mental health settings, particularly with adult populations experiencing mental health challenges, including but not limited to anxiety, depression, PTSD, and other diagnoses
  • Ability to build rapport and maintain supportive, professional relationships with adult clients navigating mental health conditions.
  • Dependable personal transportation is required.
  • Valid Washington Driver’s License and proof of automobile insurance required.
  • Must maintain case notes in a timely manner.
  • Computer literacy skills.

Responsibilities

  • Assess, orient, and facilitate the entry of individuals and families experiencing homelessness into their new homes.
  • Provide case management and develop individualized family service plans including identification of existing barriers to self-sufficiency, setting housing and income goals, and developing an action plan.
  • Evaluate progress and accomplishments of goals regularly, adjusting as needed to improve outcomes. Make referrals to appropriate internal and external providers and resources.
  • Coordinate and advocate for the involvement of appropriate professionals from other agencies.
  • Partner with Property Management staff regarding move in and departure of families and identify challenges with residents as it pertains to maintaining housing and/or abiding by the lease agreement.
  • Record case notes of all notable interactions with participants including high level conversation topics, goal setting, recommended referrals (internal and external), and overall progress.
  • Schedule and conduct community engagement activities to encourage community building.
  • Maintain accurate and on-going documentation following contract requirements, agency and department policies and procedures.
  • Respond to client emergencies.
  • Participate in and contribute to team building.
  • Attend agency and department meetings.
  • Attend inter-agency and networking meetings as appropriate.
  • Other tasks and responsibilities as assigned and required by program goals and objectives.
  • Assist with other agency requirements as directed.
