Work with the CN Guidance FACT Team to deliver services to individuals diagnosed with severe mental illness whose needs have not been met by more traditional service delivery approaches. This role involves integrating dual-recovery treatment, family support, or vocational/employment training with the tasks of other team members. The position is responsible for assessment and service planning, quality assurance and improvement processes, utilization review, incident reporting, and coordination between FACT and other systems. The Family Specialist will engage in assessments, case management, crisis intervention, and discharge planning, while maintaining case records and completing documentation in compliance with regulations. Services are provided directly, via Telehealth, face-to-face, and/or collateral services to recipients in the FACT Office or in the community. The role requires a 35-hour work week, including periodic pager coverage. The Family Specialist will assist recipients in developing appropriate expectations from treatment, strengthen their ability to understand and utilize therapy, and find resources after treatment through proper discharge planning. They will exchange information with other agencies for the recipient's benefit and ensure compliance with regulations. Utilizing and understanding the value of supervision is key to increasing practice skills, promoting better services, and maintaining accountability. The role also involves facilitating data sharing with individuals for partnered decision-making and keeping clients informed of progress, completing QI training, and cultivating a culture of inclusion. Adherence to all safety protocols and procedures is essential to ensure a safe working environment.
