About The Position

Avalon Housing is a non-profit supportive housing provider dedicated to building healthy, safe, and inclusive supportive housing communities as a long-term solution to homelessness. Supportive housing combines affordable housing with services like case management to help people not only find housing but also thrive in it. The Family Services Support Coordinator, using a Housing First approach, provides outreach, case management, and housing support services to Avalon tenants in households with minor children. These individuals often face challenges such as mental illness, substance use disorders, and chronic medical conditions in addition to chronic homelessness. The clients reside in Avalon Housing units, with private landlords, or in public housing within Washtenaw County. Support Coordinators collaborate with community partners to ensure the mental, physical, psycho-social, and housing needs of clients are met through direct service or connections to resources.

Requirements

  • Bachelor's or Master's degree in a social or behavioral science or relevant field or three years of relevant experience
  • Experience working with people who are homeless, people with mental illness, chronic health issues, and/or addiction disorders, or similar populations
  • Skilled in crisis response situations
  • Must possess a highly positive and enthusiastic attitude and strong commitment toward helping those in need
  • Proven ability to manage partnerships with a high degree of skill while upholding Avalon's mission and values, particularly when working with stakeholders who may have differing perspectives or priorities
  • Interest and experience in working with individuals who are difficult to engage and refer to traditional programs; experience with harm reduction and trauma-informed care
  • Ability to work well with diverse populations in a non-judgmental way

Responsibilities

  • Provide ongoing, active outreach and creative engagement to tenants
  • Conduct comprehensive assessments and help tenants develop action plans to achieve goals
  • Work with tenants and property management to coordinate eviction prevention efforts and develop housing permanency plans
  • Negotiate and advocate with external community resources providing medical, mental health, substance use, and other supports
  • Assist tenants with conflict resolution among tenants and neighbors
  • Work to build community and peer support among tenants, including organizing community events and activities
  • Meet documentation requirements as dictated by program need
  • Work with clients in their homes, in community centers, and in the larger community
  • Participate in on-call rotation and occasional evening and weekend work as necessary
  • Assist tenants as needed with activities of daily living such as transportation, housekeeping, meal preparation, medication, shopping, laundry, or other hands-on assistance
