About The Position

The Family Service Coordinator (FSC) is a service provider role within the NM FIT Early Intervention program. The FSC acts as a single point of contact to assist and enable families of children from birth through age three (3) to access and receive early intervention services as designated in the Individualized Family Service Plan (IFSP). This role involves coordinating, facilitating, and monitoring the delivery of services, ensuring timeliness, and coordinating across agency lines. The FSC assists parents in accessing and coordinating early intervention (EI) services and other supports identified on the IFSP, explains procedural safeguards, gathers family information, coordinates evaluations and assessments, facilitates eligibility determination, refers families to providers, schedules appointments, helps families prepare for IFSP meetings, organizes and participates in IFSP meetings, arranges and coordinates all IFSP services, conducts follow-up to ensure service delivery, coordinates funding sources, monitors service delivery, facilitates IFSP reviews, and ensures a transition plan is developed. The FSC is responsible for providing family service coordination in accordance with NM FIT Program regulations and standards, and for participating in business operating duties.

Requirements

  • A bachelor’s degree in social work, counseling, psychology, nursing, special education, early childhood education, or any other closely related field.
  • OR A bachelor’s degree in another field plus two (2) years of direct experience in serving individuals with disabilities and/or families.
  • Training for family service coordination in accordance with NM FIT Program standards within one (1) year of hire.
  • Completion of the web-based family service coordination training every three (3) years.
  • Receive reflective supervision at least once (1) every month.
  • Ability to carry out all responsibilities professionally and efficiently.
  • Ability to work in a manner that coincides with the Mission, Vision, and Values of the Company.
  • Maintain effective communications at all levels; within the work setting and company.

Responsibilities

  • Act as the single point of contact for coordinating, facilitating, and monitoring the delivery of services to ensure they are provided in a timely manner.
  • Coordinate services across agency lines.
  • Assist parents in gaining access to, and coordinating the provision of, early intervention (EI) services and other services as identified on the IFSP.
  • Explain early intervention and procedural safeguards to families.
  • Gather information from the family regarding their concerns, priorities, and resources.
  • Coordinate evaluation and assessment activities.
  • Facilitate the determination of the child’s eligibility.
  • Refer the family to providers for needed services and supports.
  • Schedule appointments for IFSP services for the child and their family.
  • Help families plan and prepare for their IFSP meeting.
  • Organize, facilitate, and participate in IFSP meetings.
  • Arrange for and coordinate all services listed on the IFSP.
  • Conduct follow-up activities to determine that appropriate services are being provided.
  • Coordinate funding sources for services provided under the IFSP.
  • Monitor the delivery of services listed on the IFSP.
  • Facilitate periodic reviews of the IFSP.
  • Ensure that a transition plan is developed at the appropriate time.
  • May be asked to coordinate the intake, evaluations and assessments, eligibility, and development of the IFSP.
  • Facilitate and participate in multidisciplinary meetings to review evaluation results and integrate findings with other disciplines.
  • Coordinate and monitor the delivery of services (including subcontractors and providers from other agencies).
  • Participate in team discussions in an articulate, positive, and productive manner.
  • Maintain equipment in proper working order.
  • Perform routine repairs.
  • Instruct staff, as assigned, in principles used in family service coordination.
  • Complete all documentation in a timely manner.
  • Attend departmental/business staff development activities.
  • Maintain proficiency in family service coordination by attending workshops, conferences, and trainings.
  • Attend all mandatory in-services and participate in Company committees as requested by supervisor.
  • May represent the Company and FSC department while participating in community outreach events, activities, meetings, trainings, etc.
  • Respond to emergency situations in a competent manner.
  • Adhere to established Company policies and procedures.
  • Assist in quality management, research, public relations, and program development projects when appropriate.
  • Will have assigned duties within Company’s Professional Development Plan.
  • May be assigned to be an FSC supervisor for other FSCs.
  • Perform other duties as assigned.
