About The Position

The Family Service Coordinator (FSC) role is part of the NM FIT Early Intervention program. The FSC acts as a single point of contact to coordinate, facilitate, and monitor the delivery of early intervention services for families with children from birth through age three. This involves assisting parents in accessing services, explaining procedural safeguards, gathering family information, coordinating evaluations and assessments, facilitating eligibility determination, referring families to providers, scheduling appointments, preparing families for IFSP meetings, organizing and participating in IFSP meetings, arranging and coordinating all services listed on the IFSP, conducting follow-up activities, coordinating funding sources, monitoring service delivery, facilitating periodic IFSP reviews, and ensuring the development of a transition plan. The FSC is responsible for providing family service coordination in accordance with NM FIT Program regulations and standards, and for participating in business operating duties.

Requirements

A bachelor’s degree in social work, counseling, psychology, nursing, special education, early childhood education, or any other closely related field.
A bachelor’s degree in another field plus two (2) years of direct experience in serving individuals with disabilities and/or families.
Training for family service coordination in accordance with NM FIT Program standards within one (1) year of hire.
Completion of the web-based family service coordination training every three (3) years.
Receive reflective supervision at least once (1) every month.

Responsibilities

Act as the single point of contact for coordinating, facilitating, and monitoring the delivery of early intervention services.
Assist parents in gaining access to and coordinating the provision of early intervention services and other services identified on the IFSP.
Explain early intervention and procedural safeguards to families.
Gather information from families regarding their concerns, priorities, and resources.
Coordinate evaluation and assessment activities.
Facilitate the determination of the child’s eligibility for services.
Refer families to providers for needed services and supports.
Schedule appointments for IFSP services for the child and their family.
Help families plan and prepare for their IFSP meeting.
Organize, facilitate, and participate in IFSP meetings.
Arrange for and coordinate all services listed on the IFSP.
Conduct follow-up activities to determine that appropriate services are being provided.
Coordinate funding sources for services provided under the IFSP.
Monitor the delivery of services listed on the IFSP.
Facilitate periodic reviews of the IFSP.
Ensure that a transition plan is developed at the appropriate time.
Coordinate the intake, evaluations and assessments, eligibility, and development of the IFSP.
Facilitate and participate in multidisciplinary meetings to review evaluation results and integrate findings with other disciplines.
Coordinate and monitor the delivery of services (including subcontractors and providers from other agencies).
Participate in team discussions in an articulate, positive, and productive manner.
Maintain equipment in proper working order and perform routine repairs.
Instruct staff, as assigned, in principles used in family service coordination.
Complete all documentation in a timely manner.
Attend departmental/business staff development activities.
Maintain proficiency in family service coordination by attending workshops, conferences, and trainings.
Attend all mandatory in-services and participate in Company committees as requested by supervisor.
Represent the Company and FSC department while participating in community outreach events, activities, meetings, trainings, etc.
Respond to emergency situations in a competent manner.
Adhere to established Company policies and procedures.
Assist in quality management, research, public relations, and program development projects when appropriate.
Have assigned duties within Company’s Professional Development Plan.
May be assigned to be an FSC supervisor for other FSCs.
Perform other duties as assigned.
