The Family Service Coordinator partners with families of eligible infants in the UNMH Newborn Intensive Care Unit (NICU) to coordinate services, facilitate IFSP development, connect families to community resources, and support positive developmental outcomes. This position promotes family engagement, informed decision-making, and access to services while ensuring compliance with New Mexico FIT Program standards and IDEA Part C regulations. Under the guidance of the Sr. Program therapist, the Family Service Coordinator will serve as the primary contact and trusted partner for families enrolled in the FIT Program, build supportive relationships with families using a strengths-based, family-centered approach, coordinate evaluations, assessments, IFSP meetings, and ongoing service delivery, connect families with medical, educational, developmental, social, and community resources, and facilitate transition from hospital to community.
