Position Summary The Family Service Coordinator (FSC) supports families of children birth to three years of age by coordinating Early Intervention services and assisting families in accessing resources, supports, and developmental services. The FSC serves as the primary point of contact for families, helping guide them through the Early Intervention process, supporting Individualized Family Service Plan (IFSP) development, coordinating services, and ensuring services are delivered effectively and timely. This position provides services primarily in home and community settings and requires collaboration with families, therapists, healthcare providers, community partners, and interdisciplinary teams.
