The mission of Family Support Services is to be the leader in providing safety, stability, and quality of life for all children by working with the community to strengthen the family unit. This role will facilitate, monitor, document, and track the Integrated Practice Team (IPT) Process. The facilitator will guide the group to brainstorm ideas that can assist a family with positive change while addressing conflict and accurately routing cases to the proper community resource. This position utilizes a family-centered and strength-based team approach with community involvement in the protection of children. The role serves as a consultant to CPI’s and CMO case managers and coordinates special case staffings. It involves networking with various community agencies and resources and is responsible for ensuring cross-system integration and interdisciplinary support. The position also participates with the Family Preservation (FPT) Team process to review referrals by a mental health provider for wraparound intervention services and assists the team with referral processing and auditing for all programs within Family Preservation.
