About The Position

The Family Practice Physician is responsible for the provision of primary care medical services in a Patient Centered Health Home context, with a focus on access to care. This role involves the direct provision of primary care medical services, meeting all established productivity expectations. The physician will provide examinations, review laboratory, referral, and imaging results to determine diagnoses, and document visits accurately using appropriate templates. The position requires accountability for budgeted visits, meeting clinical care measures, and locking records within the 72-hour window. The Family Practice Physician is responsible for meeting Family Practice Professional Focus and Goals, providing comprehensive, continuous, and coordinated medical care for acute and chronic illnesses across all ages within a family unit in a community setting. This includes health promotion, illness prevention, and applying a holistic healthcare model. The role involves managing chronic diseases and acute illnesses with broad knowledge in procedural skills, coordinating care with specialists, and utilizing community resources. The physician will advocate for access to healthcare, focusing on structured evaluation, early intervention, and health promotion. Excellent customer service and courtesy are expected in all interactions with patients, families, and staff. The provider must remain aware of wait times, provide appropriate educational materials based on patient health literacy, and be accountable for patient status as measured by clinical quality measures. Participation in daily huddles, incorporating ancillary health team members, and setting/monitoring self-management plans are key responsibilities. The physician will assist in assuring clinical procedures, maintaining up-to-date clinical protocols, and completing chart and peer reviews. They will also support performance improvement efforts, staff education, FHCN promotion, and quality improvement initiatives. Assisting in the preceptorship of mid-level practitioners and participating in the rotation of medical, physician assistant students, and dental residents are also part of the role. The physician will participate in Network Health Fairs and other community events, as well as site and provider meetings. Meeting Network and credentialing expectations for licensure and employment is required. Adherence to the Attendance and Absenteeism Policy is essential, recognizing that regular attendance is an essential function. The ability to present to and work at any FHCN location is necessary. Requirements

  • Equivalent of extensive formal training in Family Medicine, including an understanding of the application of the theory and practices of the profession to the operations of the organization, usually in the form of a Doctor of Medicine or Doctor of Osteopathic Medicine degree.
  • Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
  • Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.
  • Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions and other elements.
  • Licensed in the state of California as a Family Medicine Physician.
  • Effectively communicate professional opinions and extrapolations of information collected and synthesized/analyzed.
  • Determine appropriate methods of communicating information through the use of tables, graphs, charts and other visual forms.
  • Prepare and execute presentations to large groups.
  • Regularly required to sit and use repetitive hand movement to type and grasp.
  • Frequently required to stand or walk.
  • Occasionally lift and/or move up to 20 pounds.

Responsibilities

  • Provide comprehensive health care including health promotion and maintenance, prevention of illness and disability and application of holistic healthcare model with focus on addressing both protective factors and risk factors affecting the health and wellbeing of patient.
  • Manage chronic diseases and acute illness with broad range knowledge in procedural skills, coordination of care with specialists, and utilization of community resources when appropriate.
  • Advocate access to health care with focus of structured evaluation, early intervention and health promotion.
  • Provide excellent customer service and care.
  • Consistently behave with courtesy when interacting with patients/ family members, support staff and Network staff.
  • Remain aware of wait time during the daily workflow.
  • Provide the patient with educational materials and resources appropriate to the patient’s health literacy needs.
  • Be accountable for patient status as measured by relevant clinical quality measures.
  • Promote and participate in daily huddles.
  • Incorporate the participation of ancillary health team members in managing the care of patients / families.
  • Set and monitor self- management plans.
  • Assist in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.
  • Complete chart and peer review responsibility in a timely manner.
  • Assist the patient care team with performance improvement efforts, staff education and FHCN promotion.
  • Assist the patient care team with performance evaluations and quality improvement.
  • Assist in the preceptorship of mid-level practitioners.
  • Where relevant participate in rotation of medical, physician assistant students and dental residents.
  • Participate in Network Health Fairs & other community events.
  • Participate in site & provider meetings in a meaningful way.
  • Meet Network and credentialing expectations for licensure, and employment.
  • Adhere to the Attendance and Absenteeism Policy.
  • Present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need.
  • Perform other duties as assigned.
