The Family Practice Physician is responsible for the provision of primary care medical services in a Patient Centered Health Home context, with a focus on access to care. This role involves the direct provision of primary care medical services, meeting all established productivity expectations. The physician will provide examinations, review laboratory, referral, and imaging results to determine diagnoses, and document visits accurately using appropriate templates. The position requires accountability for budgeted visits, meeting clinical care measures, and locking records within the 72-hour window. The Family Practice Physician is responsible for meeting Family Practice Professional Focus and Goals, providing comprehensive, continuous, and coordinated medical care for acute and chronic illnesses across all ages within a family unit in a community setting. This includes health promotion, illness prevention, and applying a holistic healthcare model. The role involves managing chronic diseases and acute illnesses with broad knowledge in procedural skills, coordinating care with specialists, and utilizing community resources. The physician will advocate for access to healthcare, focusing on structured evaluation, early intervention, and health promotion. Excellent customer service and courtesy are expected in all interactions with patients, families, and staff. The provider must remain aware of wait times, provide appropriate educational materials based on patient health literacy, and be accountable for patient status as measured by clinical quality measures. Participation in daily huddles, incorporating ancillary health team members, and setting/monitoring self-management plans are key responsibilities. The physician will assist in assuring clinical procedures, maintaining up-to-date clinical protocols, and completing chart and peer reviews. They will also support performance improvement efforts, staff education, FHCN promotion, and quality improvement initiatives. Assisting in the preceptorship of mid-level practitioners and participating in the rotation of medical, physician assistant students, and dental residents are also part of the role. The physician will participate in Network Health Fairs and other community events, as well as site and provider meetings. Meeting Network and credentialing expectations for licensure and employment is required. Adherence to the Attendance and Absenteeism Policy is essential, recognizing that regular attendance is an essential function. The ability to present to and work at any FHCN location is necessary.
