About The Position

The Family Practice Physician is responsible for the provision of primary care medical services in a Patient Centered Health Home context, with a focus on access to care. This role involves the direct provision of primary care medical services meeting all established productivity expectations. The physician will provide examinations, review laboratory, referral, and imaging results to determine diagnoses, and document visits accurately using appropriate templates and recording relevant data. The position requires accountability for attaining budgeted visits, meeting clinical care measures, and locking records within the 72-hour window. The physician will provide comprehensive, continuous, and coordinated medical care for acute and chronic illnesses across all ages within a family unit in a community setting, encompassing health promotion, illness prevention, and a holistic healthcare model. Management of chronic diseases and acute illnesses requires broad knowledge in procedural skills, coordination with specialists, and utilization of community resources. The role also involves advocating for access to healthcare, providing excellent customer service, and educating patients with appropriate materials based on their health literacy needs. Promoting and participating in daily huddles, incorporating ancillary health team members, and setting/monitoring self-management plans are key aspects. The physician will also assist in assuring clinical procedures, maintaining up-to-date clinical protocols, completing chart and peer reviews, and participating in performance improvement efforts, staff education, and FHCN promotion. Assisting the patient care team with performance evaluations and quality improvement, precepting mid-level practitioners, and potentially participating in the rotation of medical, physician assistant students, and dental residents are also responsibilities. The physician will participate in Network Health Fairs and other community events, as well as site and provider meetings. Meeting Network and credentialing expectations for licensure and employment, and adhering to the Attendance and Absenteeism Policy are mandatory. The ability to present to and work at any FHCN location based on business need is required. Other duties as assigned.

Requirements

Completion of a Doctoral-level degree program with a recognized major with a minimum cumulative GPA of 2.5; or Successful attainment and maintenance of a valid, active professional license required for the role, issued by the appropriate governing or regulatory body.
Extensive formal training in Family Medicine, including an understanding of the application of the theory and practices of the profession to the operations of the organization, usually in the form of a Doctor of Medicine or Doctor of Osteopathic Medicine degree.
Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.
Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions and other elements.
Licensed in the state of California as a Family Medicine Physician.
Effectively communicate professional opinions and extrapolations of information collected and synthesized/analyzed.
Determine appropriate methods of communicating information through the use of tables, graphs, charts and other visual forms.
Prepare and execute presentations to large groups.
Regularly required to sit and use repetitive hand movement to type and grasp.
Frequently required to stand or walk.
Occasionally lift and/or move up to 20 pounds.

Manage chronic diseases and acute illness with broad range knowledge in procedural skills, coordination of care with specialists, and utilization of community resources when appropriate.
Advocate access to health care with focus of structured evaluation, early intervention and health promotion.
Provide excellent customer service and care.
Consistently behave with courtesy when interacting with patients/ family members, support staff and Network staff.
Remain aware of wait time during the daily workflow.
Provide the patient with educational materials and resources appropriate to the patient’s health literacy needs.
Achieve patient status as measured by relevant clinical quality measures.
Promote and participate in daily huddles.
Incorporate the participation of ancillary health team members in managing the care of patients / families.
Set and monitor self-management plans.
Assist in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.
Complete chart and peer review responsibility in a timely manner.
Assist the patient care team with performance improvement efforts, staff education and FHCN promotion.
Assist the patient care team with performance evaluations and quality improvement.
Assist in the preceptorship of mid-level practitioners.
Where relevant participate in rotation of medical, physician assistant students and dental residents.
