About The Position

Maple City Health Care Center (MCHCC) is a Federally Qualified Health Center dedicated to improving our community’s health by making quality comprehensive healthcare accessible to all. The Family Physician is a member of a cross-disciplinary team that includes health care providers, medical assistants, nurses, behavioral health providers, and other support staff. The Family Physician supports the mission and values of Maple City Health Care Center (MCHCC) by providing primary care to patients of all ages and genders.

Requirements

• Physician’s license to practice in the State of Indiana or ability to obtain licensure required
• DEA registration required
• State of Indiana license to prescribe controlled substances (CSR)
• Proof of residency completion with board eligibility and board certification required
• BLS required
• Ability to lift up to 10 pounds
• Ability to perform prolonged or extensive standing and walking
• Ability to perform considerable reaching, stooping, bending, kneeling, crouching
• Ability to perform prolonged periods of time in front of a computer
• We are required by federal law to verify identity and eligibility to work in the U.S.

Responsibilities

• Provides primary care for MCHCC patients in clinic
• Provides clinical leadership for the care team in which they are located
• Serves as a collaborating physician on collaborative practice agreements for nurse practitioners and midwives
• Utilizes EMR effectively in patient care
• Utilizes multiple resources for MCHCC communication
• Other duties that support our shared work, as assigned
