About The Position

Guelph Community Health Centre (GCHC) is seeking a Family Physician to join their Primary Health program in Guelph, Ontario. GCHC provides primary care services and community health programs using an interdisciplinary, team-based approach with innovative client-centered services. The organization prioritizes individuals facing barriers to health and accessing services, including those who are homeless, on low income, newcomers, have mental health or addiction issues, disabilities, are Indigenous, Black, mixed race, racialized, 2SLGBTQIA+, experiencing intersectional oppression, or are vulnerable children and their families. The clinical team includes family physicians, nurse practitioners, and registered practical nurses, working collaboratively with allied health workers such as social workers, dietitians, outreach workers, and cultural interpreters. GCHC operates within a determinants of health framework, focusing on health promotion, education, disease prevention, anti-oppression, and recognizing special risk factors affecting client health.

Requirements

  • Licensed Family Physician, as per the requirements of the Ontario College of Physicians and Surgeons, with CCFP certification
  • Possess or is eligible to obtain required malpractice coverage with CMPA
  • Experience working with communities and individuals who face barriers accessing health care services
  • A demonstrated commitment to health equity and alignment with the CHC model of care
  • Excellent communications skills, both verbal and written, along with the ability to prioritize and multi-task
  • Proficient in the use of software applications including Outlook and electronic medical record systems
  • Ability to work interdependently and respectfully in an interdisciplinary team environment

Nice To Haves

  • Previous experience in community health/social services and/or a values-based organization is an asset

Responsibilities

  • Provide comprehensive primary care services, including assessment and treatment, preventative care, supportive client counseling, prenatal and neonatal care, and clinical consultation
  • Work within a determinants of health and equity framework, emphasizing health promotion and education, and disease prevention
  • Participate in GCHC on call service, as required, in rotation
  • Ensure high quality and safe care is delivered by maintaining complete and accurate medical records, participating in case conference and chart reviews and supporting the implementation of new guidelines and directives
