Requirements
- Any state medical license
- Board Certification - Family Medicine, ABFM or AOBFM
- Family Medicine Residency
- Possess a minimum of 1 year of post-residency clinical experience within the last 3 years providing direct patient care in Family Medicine
- DEA and Controlled Substance License
- Current BLS, ACLS, NRP, PALS
