About The Position

HealthOne Medical & Wellness is a multidisciplinary health and wellness clinic with 8 departments, including Medical, Dental, Rehab, Optometry, Wellness, Mental Health, Skin Clinic, and Pharmacy services across North York, Toronto Harbourfront, and King West. The clinic aims to deliver holistic healthcare and help Canadians live healthy and inspired lives. HealthOne fosters a collaborative, innovative, and inspiring environment, investing in its people and offering growth opportunities. This role is for a Family Physician to join the North York clinic, build a long-term family practice with clinic support, and play a key role in growing longitudinal family medicine towards a new FHO+ model. The practice will initially focus on walk-in medicine while building towards the FHO+ model with a growing emphasis on longitudinal care.

Requirements

Patient focused and service minded approach to delivering excellent, comprehensive care.
Empathetic, collaborative approach and ability to work effectively with a wide range of internal and external stakeholders.
Commit to working at HealthOne for a minimum 4 days per week + 1 weekend day per month, with reliable scheduling.
Licensed to practice as a Family Physician in Ontario and in good standing with the CPSO.

Nice To Haves

1+ year of experience preferred, however new grads are also welcomed.

Responsibilities

Build strong relationships with patients and provide an exceptional care experience, with a focus on compassion and clinical excellence.
Support patients with their acute care, preventative care, and chronic disease management needs with clear education, shared decision making, and appropriate continuity.
Apply full scope to create thorough diagnoses and treatment plans which utilize the multidisciplinary team to support patients with all interconnected aspects of their health journey.
Engage with the multidisciplinary team and internal stakeholders through on-site education sessions and care innovation initiatives across HealthOne.
