About The Position

Nova Scotia Health is seeking family physicians to join a new and developing team at the Lunenburg County Primary Care Clinic (PCC) in Bridgewater, Nova Scotia. The clinic provides temporary primary care access to patients registered on the Need a Family Practice registry and living in the Lunenburg County area. Patients can make appointments for immediate healthcare needs while remaining on the registry to find a permanent family practice. The clinic operates collaboratively with family physicians, nurse practitioners, nurses, a care team assistant, a social worker, and a pharmacist. It is managed by Nova Scotia Health with administrative support and is fully integrated on the Med Access EMR system. The facility is well-maintained and includes a meeting space and free on-site parking. The clinic will also work with community partners to deliver health education aligned with Primary Health Care principles.

Requirements

  • Eligible for licensure with the College of Physicians and Surgeons of Nova Scotia (CPSNS).
  • Eligible for certification with the College of Family Physicians of Canada (CFPC).
  • Membership with the Canadian Medical Protective Association (CMPA).

Responsibilities

  • Ensure the delivery of comprehensive primary health care services to patients of all ages and stages of health or illness.
  • Be accountable for appropriate workload and practice.
  • Provide continuum of patient care with health care providers within your practice and/or community.
  • Provide care and coordination of community-based care, secondary and tertiary level health care.
  • Operate within the Bylaws and Rules and Regulations of Nova Scotia Health Authority.
  • Work in collaboration with other team members in the practice to best suit the community’s health needs.
  • Work with other providers to provide after-hours access and emergency coverage when required.
  • Participate in quality improvement and evaluation activities.
  • Utilize an electronic medical record (EMR) and record all relevant patient details in a provincially approved EMR which is accessible to all collaborating team members.
