About The Position
Our client, a respected regional healthcare organization, is seeking two Family Nurse Practitioners to join its outpatient primary care team. Requirements
- Licensed Family Nurse Practitioner (FNP).
- Active RN license or eligibility for RN licensure in the State of New York.
- Strong patient-centered approach with excellent communication and clinical decision-making skills.
Nice To Haves
- At least one year of experience is preferred in primary care, family medicine, internal medicine, or community health.
Responsibilities
- Provide comprehensive primary care services across all stages of life in an outpatient setting.
- Diagnose, treat, and manage acute and chronic health conditions.
- Perform preventive care, wellness visits, patient education, and chronic disease management.
- Build lasting relationships with patients and families through continuity of care.
- Collaborate with physicians, advanced practice providers, nursing staff, and interdisciplinary teams.
- Maintain accurate clinical documentation using the electronic medical record (EMR).
- Participate in quality improvement initiatives and promote evidence-based care.
