About The Position

The Center for Human Development (CHD) is seeking a Family Nurse Practitioner to join their team in Easthampton, MA. This is a full-time, no on-call position with a salary range of $135,000 – $165,000. This new role is designed around provider sustainability and clinical quality, allowing the practitioner to build patient relationships from day one without inheriting a pre-existing panel or backlog. The schedule is Monday–Friday only, with no evenings, weekends, or routine on-call duties. Protected time for administrative tasks and charting is built into the workday. The role involves community-based care, providing comprehensive primary care across the lifespan, including chronic disease management and preventative education. The practitioner will have the autonomy to practice evidence-based medicine within a supportive, multidisciplinary regional care team, with dedicated MA/RN support, embedded behavioral health colleagues, and visible medical leadership.

Requirements

• Master’s or Doctoral degree with FNP Certification.
• Valid MA APRN license.
• Strong judgment.
• Passion for community-based, patient-centered care.
