About The Position

The Family Nurse Practitioner (FNP) Float provides evidence-based primary and family care to a panel of patients while collaborating with a dedicated clinical care team. The team works with the provider daily to improve outcomes across the panel. Care includes assessing and diagnosing patients, treating and managing health problems, providing health counseling, and planning dispositions. The role reports to the practice manager. The position also involves clinic administration, including opening and closing the clinic, managing inventory, following up with patients, verifying insurance, collecting payments, and maintaining a clean environment. The practitioner handles appointments and routine examinations, discusses healthy lifestyle choices, conducts diagnostic tests, and coordinates with other healthcare professionals to support comprehensive care. The practitioner encourages patients to participate in their healthcare through health screenings, behavioral health and wellness services, and chronic condition management.

Requirements

Verifiable high school diploma or GED.
At least 1 year of recent primary care experience as a Nurse Practitioner in family medicine, a primary care practice, or urgent care.
Working knowledge and understanding of quality measurement in chronic disease management.
Self-motivated; able to prioritize and solve problems, take initiative, and advocate for patients and the practice.
Effective verbal, written, and electronic communication skills.
Outstanding organizational skills and ability to multitask.
Initiative, problem-solving ability, adaptability, and flexibility.
Ability to work remotely within a clinical care team culture.
Ability to work without direct supervision and practice autonomously.
Proficiency with information management and technology.
Master's degree from a Family Nurse Practitioner program, current National Board Certification, and a license to practice in the Advanced Practice Nurse role in the state of employment.

Responsibilities

Provide evidence-based primary and family care, including assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning.

Complete diagnostic tests to help diagnose and treat injuries or illnesses.
Coordinate with other healthcare professionals to maintain a comprehensive plan of care.
Encourage patients to participate actively in their healthcare and well-being through health screenings, behavioral health and wellness, and chronic condition management.
