The Family Nurse Practitioner (FNP-C or FNP-BC) maintains an independent clinical practice under the direct supervision of the Chief Medical Officer, as a member of the SHHC’s clinical team, responsible for office visits, mobile medical unit visits, and home visits as appropriate to meet patient needs. It is expected that the Nurse Practitioner will care for patients of all ages, from newborn through geriatric in all SHHC locations. In addition to a general practice, Nurse Practitioner may provide routine prenatal and gynecological care to SHHC patients, not including deliveries, and will work with SHHC’s contract OB/GYN and prenatal team to ensure delivery of highest quality care to patients enrolled in the prenatal program. The Nurse Practitioner is a key member of the FQHC care team working to provide quality comprehensive services to patients and will work collaboratively with staff to provide team-based coordinated care. The ideal candidate has strong clinical experience providing comprehensive primary care services across the lifespan, with an emphasis on diverse adult, geriatric, and family medicine populations; and is passionate about serving medically underserved, diverse communities in a mission-driven community health setting.
