About The Position

The PRN Family Nurse Practitioner (FNP) serves as a provider of primary health care services, coordinating and referring clients for health care and health education while delivering comprehensive, patient-centered care. This position provides clinical coverage on an as-needed basis and supports continuity of care in accordance with clinic needs, professional standards, and applicable licensing requirements. This position functions in a PRN (as-needed) capacity, with scheduling and hours determined by clinic needs and patient care demands.

Requirements

  • Graduation from an accredited Nurse Practitioner program with certification as a Family Nurse Practitioner (FNP) or related primary care specialty.
  • Nurse Practitioner, unencumbered licensure with State of Oregon as Certified Nurse Practitioner and prescriptive authority.
  • Current DEA registration or ability to obtain upon hire.
  • Basic Life Support within introductory period.
  • Evidence of experience sufficient for successful performance of duties noted above.
  • Must pass a pre-employment or for cause drug tests.
  • Must pass criminal background and DMV checks.

Nice To Haves

  • Experience working within the Native American/Alaska Native community
  • Prior experience in primary care, family practice, community health, or federally qualified health center setting.

Responsibilities

  • Provide comprehensive primary health care services on an as-needed basis to support clinic operations, including provider absences and fluctuations in patient demand.
  • Coordinate patient care with other providers and agencies and advocate for clients within health care systems.
  • Provide services in prenatal integrated health care of high-risk chemically dependent pregnant patients, in consultation and/or collaboration with obstetrical health care providers.
  • Perform initial and ongoing health risk assessments and counseling for high-risk lifestyle behaviors and/or familial diseases, including ongoing drug and alcohol assessment and HIV screening.
  • Encourage family participation in the health care of patients as deemed appropriate by the patient and other clinic providers.
  • Develop and document plans of care and outcomes evaluations.
  • Coordinate with contractual pharmacy, primary care providers, hospitals, and laboratories for required diagnostic procedures and delivery/births.
  • Provide health education to individual patients as needed, including birth control, safer sex, high-risk behaviors, and associated health consequences.
  • Coordinate the care patients receive from other providers and agencies and make referrals as appropriate.
  • Engage in ongoing education and specialized training in order to maintain a high quality of care to the Clinic's patient population.
  • Attend meetings and participate in clinic activities as requested, as needed.
  • Document patient care accurately and appropriately in accordance with organizational policies and professional standards.
