About The Position

Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Family Nurse Practitioner in Page, Arizona. This is an outpatient Family Nurse Practitioner role serving patients across the lifespan within an underserved community healthcare setting. It offers the opportunity to practice evidence-based medicine within a collaborative multidisciplinary care environment. The role features a team-focused clinical structure supporting preventative care, chronic disease management, and long-term patient relationships. There is built-in administrative time and manageable patient volume designed to support provider balance and quality care delivery. The position is located in a scenic Northern Arizona location near Lake Powell, Antelope Canyon, and Horseshoe Bend.

Requirements

• Board Certified Family Nurse Practitioner (FNP-C or FNP-BC) REQUIRED
• Arizona license or ability to obtain REQUIRED
• Active DEA registration REQUIRED
• Current BLS certification REQUIRED
• Passion for underserved and culturally diverse communities REQUIRED
• Strong interdisciplinary collaboration skills REQUIRED

Nice To Haves

• Two years ambulatory care experience preferred
• Navajo or Spanish language skills preferred

Responsibilities

• Deliver outpatient primary care services for patients of all ages
• Manage acute and chronic medical conditions within community health environment
• Collaborate with interdisciplinary healthcare teams to coordinate patient care
• Maintain evidence-based preventative care and treatment standards
• Support underserved and culturally diverse patient populations
• Maintain clinical documentation and care coordination workflows
