About The Position

A well-established, not-for-profit community hospital is seeking a Family Nurse Practitioner (FNP) to join its outpatient primary care practice in Southbury, Connecticut. Candidates with a strong background in primary care, chronic disease management, and preventive medicine will be considered. This is a full-time, permanent outpatient-focused position providing comprehensive care to a diverse patient population across the lifespan, with the opportunity to manage a wide range of conditions including diabetes, hypertension, asthma, and other chronic and acute illnesses. Both experienced nurse practitioners and new graduates are encouraged to apply. This organization is a 160-bed acute care hospital serving a large regional population and is nationally recognized for its patient-centered approach to care. Requirements

  • Master’s or Doctorate degree in Nursing from an accredited program
  • Board Certified as a Family Nurse Practitioner
  • Active Connecticut APRN license (or eligibility)
  • Strong communication and clinical decision-making skills

Responsibilities

  • Provide comprehensive primary care services, including evaluation, diagnosis, and treatment
  • Conduct routine physical exams, preventive screenings, and health assessments
  • Manage acute and chronic conditions such as diabetes, hypertension, asthma, and infections
  • Order and interpret diagnostic tests including labs and imaging
  • Prescribe medications and develop patient-centered treatment plans
  • Educate patients on disease prevention, lifestyle changes, and overall wellness
  • Collaborate with physicians and multidisciplinary teams for coordinated care
  • Maintain accurate and timely EMR documentation
  • Participate in quality improvement and patient care initiatives
