About The Position
A well-established, not-for-profit community healthcare organization is seeking a Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join its outpatient primary care practice in Southbury, Connecticut. This is a full-time, permanent opportunity focused on delivering high-quality, patient-centered care in a collaborative clinical environment. The ideal candidate will have a strong foundation in primary care, preventive medicine, and chronic disease management. Requirements
- Master’s or Doctorate degree in Nursing from an accredited program (for Nurse Practitioners)
- Board Certification as a Family Nurse Practitioner (FNP) (for Nurse Practitioners)
- Active Connecticut APRN license (or eligibility) (for Nurse Practitioners)
- Master’s degree from an accredited Physician Assistant program (for Physician Assistants)
- NCCPA certification (for Physician Assistants)
- Active Connecticut Physician Assistant license (or eligibility) (for Physician Assistants)
- Strong interest or experience in outpatient primary care
- Excellent clinical judgment, communication, and patient care skills

Responsibilities
- Provide comprehensive outpatient primary care services, including evaluation, diagnosis, and treatment
- Conduct routine physical examinations, preventive screenings, and health assessments
- Diagnose and manage acute and chronic conditions, including diabetes, hypertension, asthma, infections, and other medical concerns
- Order, review, and interpret diagnostic testing, including laboratory studies and imaging
- Prescribe medications and develop individualized treatment plans
- Educate patients regarding disease prevention, wellness, and lifestyle modifications
- Collaborate with physicians, specialists, and multidisciplinary healthcare teams to ensure coordinated patient care
- Maintain accurate and timely documentation within the electronic medical record (EMR) system
- Participate in quality improvement initiatives and population health programs
- Support the delivery of evidence-based, patient-focused healthcare services
