We are seeking a board-certified Family Nurse Practitioner for outpatient primary care in Woodsville, NH. Requirements
- Active New Hampshire APRN License (or eligible)
- Board Certified as a Family Nurse Practitioner (FNP-BC or FNP-C)
- 3 Years of Direct Primary Care Experience
- Experience with Electronic Health Records (AthenaOne Preferred)
Responsibilities
- Provide Outpatient Primary Care to Patients Across the Lifespan
- Manage Preventive Care, Chronic Conditions, and Acute Visits
- Order and Interpret Labs and Diagnostic Studies
- Prescribe and Manage Medications
- Collaborate with an Integrated Team of Physicians, Advanced Practice Providers, and Behavioral Health Professionals
- Participate in Provider Mentorship, Quality Initiatives, and Organizational Decision-Making
- Complete Timely Documentation in AthenaOne
