About The Position

The Edward M. Kennedy Community Health Center is seeking a Family Nurse Practitioner to join their Urgent Care team. This role involves providing high-quality patient care within the PCMH model, collaborating with various disciplines such as mental and behavioral health, pharmacy, Medication Assisted Treatment (MAT) program, Reproductive Health, Gender Affirming Care, Dental, and Optometry. The position offers the opportunity to care for patients across the entire lifespan, including infants and pregnant patients, and to promote continuity of care with the primary care team. Requirements

  • 3+ years of experience.
  • Completed Master’s level Nursing Practice Degree and Family Nurse Practitioner Certification
  • FNP Board Certification
  • Active, Unrestricted FNP License in Massachusetts.
  • MCSR and DEA

Responsibilities

  • Provides medical care to patients with acute medical problems or acute exacerbations of chronic illnesses.
  • Examines patients, formulates diagnostic plans, and orders diagnostic testing.
  • Interprets examination findings and test results and implements treatment plans.
  • Provides acute care for patients across the entire lifespan, including infants and pregnant patients.
  • Determines the need for consultation and assists in medical care and treatment provided by other specialists.
  • Participates in other care procedures according to training and demonstrated ability.
  • Prescribes and assesses effectiveness of pharmaceuticals, other medications and treatment regimens as appropriate for patient’s medical conditions.
  • Promotes continuity for patients with the primary care team within EMK and facilitates appropriate follow up with primary care team when appropriate.
  • Meets the Health Center’s annual productivity goal.
  • Maintains and updates Electronic Health Records (EHR) in a systematic format and completes required documentation within 48 hours.
