About The Position

The Family Nurse Practitioner- Primary Care role involves providing comprehensive healthcare services within a primary care setting. This position requires obtaining patient histories, performing physical examinations, and diagnosing and managing acute and chronic health conditions according to evidence-based guidelines. The practitioner will order and interpret diagnostic studies, prescribe medications, and perform various diagnostic and therapeutic procedures. Key responsibilities include promoting self-care, health promotion, and disease prevention through education and counseling, referring patients to appropriate care sources, and collaborating with physicians and the interprofessional team. The role also focuses on ensuring patient satisfaction, efficient resource utilization, supporting the primary care physician-patient relationship, and actively establishing primary care relationships for new patients. Communication, documentation, and participation in community outreach are also integral aspects of this position. The role contributes to achieving national benchmarks for quality care, cost containment, and reducing patient length of stay, particularly in collaboration with hospitalist teams. Discharge and transfer planning, and responding to the Emergency Department for patient evaluation and treatment are also included.

Prescriptive authority includes controlled substance prescriptions in compliance with state regulations. Collaborative physician providers and APNs/PAs will meet to discuss patient care and review current literature. Prescribing practices will be evaluated through random review by collaborative physicians. The role requires demonstrating knowledge and skills appropriate for the age of patients served, understanding growth and development principles, and assessing data to identify age-specific needs.

Requirements

• Master’s degree with advanced training or education as an Advanced Practice Nurse licensed to practice in the state of employment and appropriate Board certifications maintained.
• Valid Advanced Practice Nurse license in the state of employment and all applicable state(s) providing care.
• Required and Board Certifications as required for credentialing.
• Required and Current Basic Life Support (BLS) for Healthcare Providers certification.
• Ability to learn and use electronic medical record efficiently and effectively.
• Demonstrated flexibility to work on an interdisciplinary team; ability to work in various roles and capacities and to respect boundaries in each role.

Responsibilities

• Obtains histories and performs physical examinations.
• Diagnoses and manages acute and chronic health conditions via evidence-based guidelines.
• Orders, conducts, and interprets diagnostic studies and laboratory tests.
• Prescribes or recommends pharmacologic and non-pharmacologic therapies, including controlled medications.
• Performs diagnostic and therapeutic procedures including suturing, wound care, incision and drainage, splinting, casting, biopsies, and invasive procedures.
• Assists patients to maximize their abilities to cope with health and illness.
• Promotes self-care, health promotion, and disease prevention through education and counseling.
• Refers patients to appropriate sources of care.
• Consults appropriate physician(s) for patient care needs.
• Maintains a collegial relationship with the interprofessional team.
• Takes responsibility for assuring patient/family satisfaction with care provided.
• Presents and documents patient data.
• Assures the efficient utilization of resources while supporting clinical pathway implementation and compliance.
• Supports the primary care physician/patient relationship.
• Actively establishes primary care relationships for patients who do not have primary care physicians.
• Participates in the assurance of robust communication systems for patient updates, discharge summaries, etc.
• Assists with relevant billing documentation and development of efficient billing processes.
• Develops and participates in community outreach programs/activities.
• Works to exceed national benchmarks for quality care delivery, cost containment and length of stay.
• Responsible with hospitalist team to help reduce patient length of stay by performing tasks and evaluations in a timely manner, as well as, advocating for the patient regarding system issues.
• Assures appropriate and timely referrals to an appropriate facility.
• Plans/coordinates patient discharge and/or transfer.
• Responds to the Emergency Department for evaluation and treatment of patients.
