About The Position

At Via Care Community Health Center (VCCHC), our mission is to provide exceptional health care services to patients throughout their lives, with a vision of ensuring that affordable, high-quality care is accessible to all. The Family Nurse Practitioner or Physician’s Assistant (FNP/PA) will work collaboratively with physicians and other health care professionals to provide comprehensive medical services to patients regardless of age or gender (as allowed by training/ experience). This includes preventive care, immunizations, primary care and treatment, health education, family planning, reproductive health services, physical examinations, etc. The FNP/PA will practice under a supervising physician. This is a Full-Time position, working Tuesday - Saturday.

Requirements

  • Valid State of California Medical License (PA/FNP)
  • Valid DEA License
  • Valid Certificate of Professional Liability Insurance
  • English / Spanish Bilingual
  • Must have a reliable automobile for use on the job
  • Subject to a criminal background check prior to employment
  • Must possess valid California Driver License
  • Must have and maintain State required automobile insurance coverage
  • TB and post-employment physical clearance, to be renewed every year

Responsibilities

  • Performs medical histories, physical exams, assessment, and planning of clinic patients based on chief complaint and/or health status and document Health Care Maintenance status as per established protocols.
  • Carries out mission, vision and values established by the CEO/Chief Medical Officer and Board of Directors
  • Perform and or assist in the performance of laboratory and screening procedures delegated by the supervising physician
  • Perform or assist in the performance of the therapeutic procedure delegated by the supervising physician
  • Recognize and evaluate situations, which call for immediate attention of the physician
  • Instruct and counsel patients regarding matter pertaining to their physical and mental health
  • Facilitate patient referrals to the appropriate health facilities, agencies, and resources of the community
  • Review treatment and therapy plans, order routine diagnostic laboratory tests and procedures, and routine diagnostic radiological services
  • Performs other duties as assigned by supervisor
