About The Position

Colusa Medical Center (CMC) is seeking a Physician Assistant or Family Nurse Practitioner (NP, PA) to serve the primary care needs of its local rural community residents in Glenn county and nearby areas, California. CMC is a hospital-based system providing emergency, acute, and rehabilitative care, as well as Rural Health Clinics offering primary family practice and specialty care. The role involves assuming primary health care for specific patients under the direction of the Medical Director, adhering to established standards and providing patient care directions to staff. The center is dedicated to improving healthcare quality, increasing accessible and affordable services, maintaining public trust, sharing resources, and promoting disease prevention and individual responsibility.

Requirements

Current BLS required.
Current NP or PA license in California (Required)
Current DEA license

Nice To Haves

ACLS certification preferred.

Responsibilities

Provide in-person patient care, including assessments, diagnosis, treatment planning, and follow-up.
Conduct visits (triage, follow-ups, chronic condition support, patient education).
Support patient access by managing urgent issues and communicating care plans clearly.
Provide primary clinical care of clients of all ages in accordance with standardized procedures.
Develop a problem list, a diagnostic, therapeutic and teaching plan, which includes preventive health.
Patient education and counseling.
Furnish and adjust medications in the formulary in accordance with the law.
Order diagnostic studies such as laboratory and radiologic tests, as needed.
Collaborate closely with physicians, MAs, and administrative staff to ensure seamless patient care.
Maintain accurate, timely documentation in the EHR. Fills out all necessary forms.
Participate in team huddles, quality initiatives, and workflow improvement efforts.
Refer patients for specialty evaluation and care as needed.
Review patient’s records, including laboratory data, for abnormalities and completeness and is responsible for providing appropriate follow-up.
Oversee provider's inbox items including medication refill requests, lab/test results, and care-coordination tasks.
Respond to or route items appropriately to ensure timely, accurate resolution.
Escalate clinical concerns to supervising physicians when needed.
