About The Position

This is an opportunity to become an early clinical leader in a growing geriatric healthcare organization addressing a significant shortage of care throughout Lake Havasu and the surrounding region. Many older adults in this community have difficulty accessing consistent primary care. Some must travel hours for appointments, while others lack a dependable provider who understands their medical history, living environment, family dynamics, and long-term goals. As an FNP or AGNP, you will provide comprehensive primary care to older adults living in assisted living, memory care, and independent living communities. You will build lasting relationships with patients, families, facility leaders, caregivers, and community partners while helping establish a higher standard of accessible, responsive geriatric care. The organization’s care philosophy is informed by person-centered and Montessori-based principles. Patients are treated as individuals with purpose, preferences, abilities, and meaningful lives, not simply as diagnoses or a collection of medical conditions. The initial month will focus heavily on training, onboarding, credentialing, documentation, billing processes, clinical expectations, community introductions, and preparation to begin building the patient panel. Months Two Through Six will involve progressively increasing patient visits and community rounding across assisted living, memory care, and independent living communities. A typical day may eventually include approximately 7 to 12 patient interactions, depending on patient complexity, travel, telehealth visits, care coordination, and administrative responsibilities. During this launch period, patient and community needs may require availability across six days and participation in an on-call schedule. Responsiveness to urgent patient, family, and facility concerns will be essential. After the initial ramp-up, leadership will evaluate patient volume, workload, scheduling, and the provider’s professional goals with the intention to create a sustainable practice model that offers meaningful autonomy, flexibility, and room for continued growth. Your day will be varied and highly relationship-driven. Rather than practicing from one traditional clinic location, you will travel among senior living communities to care for patients where they live. You may begin the day rounding at a memory care community, meet with a family concerned about a change in a patient’s behavior, coordinate with facility staff regarding a medication adjustment, complete follow-up visits at an assisted living community, and conduct telehealth or care-coordination work later in the day. You will manage the full spectrum of geriatric primary care, including chronic disease, preventive care, acute concerns, medication management, cognitive decline, changes in function, and transitions between healthcare settings. Many patients will have multiple chronic conditions and complex medication regimens. Strong clinical judgment is important, but success also requires patience, time management, emotional intelligence, thorough documentation, and the ability to communicate clearly with families and caregivers.

Requirements

• Licensed Nurse Practitioner with FNP or AGNP preparation
• Current Arizona license or eligibility to obtain Arizona licensure
• DEA registration or ability to obtain
• Strong clinical decision-making and critical-thinking skills
• Excellent interpersonal and communication skills
• Ability to practice independently and collaboratively
• Strong documentation and time-management habits
• Reliable transportation and willingness to travel throughout the local service area
• Licensed Nurse Practitioner with FNP or AGNP preparation and eligibility to practice in Arizona
• DEA registration or ability to obtain
• Reliable transportation for local travel between facilities

Responsibilities

• Provide comprehensive primary care to older adults living in memory care, assisted living, and independent living communities
• Evaluate and manage chronic diseases, acute concerns, cognitive changes, and medically complex geriatric conditions
• Conduct preventive care, routine assessments, and follow-up visits
• Develop individualized care plans focused on health, function, independence, dignity, and quality of life
• Manage medications and reduce unnecessary polypharmacy when clinically appropriate
• Identify changes in patient condition and respond before they become larger medical problems
• Communicate promptly with patients, families, caregivers, and facility leadership
• Collaborate with physicians, nurses, therapists, specialists, hospice providers, home health organizations, and other community partners
• Coordinate care across senior living communities, hospitals, specialists, and other healthcare settings
• Educate patients and families regarding diagnoses, medications, treatment plans, and goals of care
• Maintain accurate, individualized, complete, and timely documentation
• Support appropriate billing, coding, compliance, and quality standards through thorough clinical notes
• Participate in the practice’s on-call coverage model
• Help develop efficient workflows and contribute ideas as the organization grows
• Exercise independent clinical judgment while seeking physician collaboration when appropriate
• Represent the organization professionally within senior living communities and throughout the Lake Havasu region
