The HMG Family Navigator I representative serves as a primary point of access for families through the Help Me Grow Solano Program, by responding to inquiries, assessing needs, helping navigate and connect to appropriate services throughout Solano County. The ideal candidate is compassionate, organized, and service-oriented, with ability to build trust and rapport with families. They are effective communicators with strong listening, written, and verbal skills, and able to manage multiple tasks in a fast-paced environment.

Requirements

  • Excellent listening skills and the ability to ask inquiring questions, identify and understand concerns, and identify appropriate referral process
  • Must be flexible and adaptable
  • Excellent organization skills and ability to meet timely follow up requirements
  • Must possess a professional and friendly attitude and be able to quickly develop a rapport with clients over the phone and support fellow teammates
  • Strong notetaking ability for training and documenting conversation with clients either in person or over the phone
  • Ability to learn and navigate new software quickly
  • Familiar with and the ability to use computers for data entry and word processing
  • Proficient in Microsoft Office products
  • Ability to attend off-site training, meetings, workshops, etc.
  • Ability to lift at least 25 pounds
  • CA Driver License and dependable automobile with appropriate insurance coverage.
  • A minimum of two year’s experience using a computer database program, internet experience, or equivalent combination of related education and experience

Nice To Haves

  • Minimum one year’s experience working with families, in a social service agency, call center or similar community resource service preferred
  • A.A. degree in Child Development or E.C.E., or at least 12 Core Units of Early Childhood Education/Child Development preferred
  • Bilingual Spanish required

Responsibilities

  • Provide case management services, monitor referrals, develop family support plans, document case notes, and conduct required follow-up within established timelines
  • Assess family needs and coordinate appropriate referrals to community resources and developmental services
  • Maintain accurate and timely client records, data entry, tracking, and reporting in compliance with program procedures
  • Respond to and inquiries promptly and professionally
  • Maintain current knowledge of community resources and build collaborative relationships with service providers and partners
  • Safeguard confidential information and comply with all privacy and mandated reporting requirements
  • Demonstrate strong communication, customer service, organization, and problem-solving skills
  • Collaborate with team members, participate in outreach events, and contribute to program improvement initiatives
  • Represent SFCS and HMG in the community by sharing information about services and promoting developmental screening and family support programs
  • Support and promote the Lending Library, and provide support to walk-in clients
