About The Position

The Family Practice Physician is responsible for the provision of primary care medical services in a Patient Centered Health Home context, with a focus on access to care. This role involves the direct provision of primary care medical services, meeting all established productivity expectations, including examination, reviewing results, and documenting visits accurately. The physician will manage acute and chronic illnesses, promote health, and advocate for access to healthcare. Excellent customer service and patient education are paramount. The role also includes participation in performance improvement efforts, staff education, and adherence to network and credentialing expectations.

Requirements

  • Completion of a Doctoral-level degree program with a recognized major with a minimum cumulative GPA of 2.5; or Successful attainment and maintenance of a valid, active professional license required for the role, issued by the appropriate governing or regulatory body.
  • Performance of the duties and responsibilities of the job requires the equivalent of extensive formal training in Family Medicine, including an understanding of the application of the theory and practices of the profession to the operations of the organization, usually in the form of a Doctor of Medicine or Doctor of Osteopathic Medicine degree.
  • Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
  • Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.
  • Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions and other elements.
  • Licensed in the state of California as a Family Medicine Physician.
  • Effectively communicate professional opinions and extrapolations of information they collect and synthesize/analyze.
  • Determine appropriate methods of communicating information through the use of tables, graphs, charts and other visual forms.
  • Preparation and execution of presentations to large groups.
  • Sit and use repetitive hand movement to type and grasp.
  • Frequently required to stand or walk.
  • Occasionally lift and/or move up to 20 pounds.

Responsibilities

  • Direct provision of primary care medical services meeting all established productivity expectations.
  • Provide examination and review appropriate laboratory, referral and imaging results to determine a diagnosis.
  • Document visits accurately utilizing appropriate templates, and record relevant data to attain a diagnosis.
  • Attain budgeted visits.
  • Meet clinical care measures.
  • Lock records in the 72-hour window.
  • Provide comprehensive, continuous and coordinated medical care for acute and chronic illness at all ages within the context of family unit in a community setting.
  • Provide comprehensive health care including health promotion and maintenance, prevention of illness and disability and application of holistic healthcare model with focus on addressing both protective factors and risk factors affecting the health and wellbeing of patient.
  • Manage chronic diseases and acute illness with broad range knowledge in procedural skills, coordination of care with specialists, and utilization of community resources when appropriate.
  • Advocate access to health care with focus of structured evaluation, early intervention and health promotion.
  • Provide excellent customer service and care.
  • Consistently behave with courtesy when interacting with patients/ family members, support staff and Network staff.
  • Remain aware of wait time during the daily workflow.
  • Provide the patient with educational materials and resources appropriate to the patient’s health literacy needs.
  • Accountable for patient status as measured by relevant clinical quality measures.
  • Promote and participate in daily huddles.
  • Incorporate the participation of ancillary health team members in managing the care of patients / families.
  • Set and monitor self-management plans.
  • Assist in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.
  • Complete chart and peer review responsibility in a timely manner.
  • Assist the patient care team with performance improvement efforts, staff education and FHCN promotion.
  • Assist the patient care team with performance evaluations and quality improvement.
  • Assist in the preceptorship of mid-level practitioners.
  • Where relevant participate in rotation of medical, physician assistant students and dental residents.
  • Participate in Network Health Fairs & other community events.
  • Participate in site & provider meetings in a meaningful way.
  • Meet Network and credentialing expectations for licensure, and employment.
  • Adhere to the Attendance and Absenteeism Policy.
  • Ability to present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need.
  • Perform other duties as assigned.
