As a Family Medicine Physician (MD/DO), you will help individuals and families access high-quality, comprehensive healthcare by providing preventive, acute, and chronic disease management services that improve health outcomes and strengthen the communities we serve. Your work will directly support improved patient outcomes, better access to care, and the overall health and stability of the communities we serve. This role offers the opportunity to practice meaningful community medicine in a mission-driven healthcare environment where your clinical expertise directly impacts patients who may otherwise face barriers to care. Requirements
- Current Ohio license to practice medicine (MD or DO)
- Board Certified or provide letter indicating qualification to sit for certification with expectation of obtaining certification within one year.
- Valid Driver’s License

Responsibilities
- Provide comprehensive primary care services for patients across the lifespan, including preventive care, acute illness management, and chronic disease management.
- Conduct patient assessments, diagnose conditions, develop treatment plans, prescribe medications, and coordinate referrals as needed.
- Build strong relationships with patients through education, counseling, and shared decision-making.
- Collaborate with an interdisciplinary healthcare team to support integrated, whole-person care.
- Maintain accurate and timely documentation while supporting quality measures, patient safety, and regulatory requirements.
- Participate in quality improvement initiatives, peer review, and clinical activities that enhance patient care.
