About The Position

The Family Practice Physician is responsible for the provision of primary care medical services in a Patient Centered Health Home context, with a focus on access to care. This role involves the direct provision of primary care medical services, meeting all established productivity expectations. The physician will provide examinations, review laboratory, referral, and imaging results to determine diagnoses, and document visits accurately using appropriate templates. The position requires comprehensive, continuous, and coordinated medical care for acute and chronic illnesses across all ages within a family unit in a community setting. This includes health promotion, illness prevention, and applying a holistic healthcare model. The physician will manage chronic diseases and acute illnesses, demonstrating broad knowledge in procedural skills, coordinating care with specialists, and utilizing community resources. A key aspect of the role is advocating for access to healthcare, focusing on structured evaluation, early intervention, and health promotion. Excellent customer service and courtesy are expected in all interactions with patients, families, and staff. The physician will provide patients with appropriate educational materials and resources based on their health literacy needs and will promote and participate in daily huddles. Collaboration with ancillary health team members in patient care management, setting and monitoring self-management plans, and assisting in the assurance of clinical procedures and maintenance of up-to-date clinical protocols are also essential. The role includes chart and peer review responsibilities, assisting with performance improvement efforts, staff education, and FHCN promotion. Additionally, the physician will assist the patient care team with performance evaluations and quality improvement, and may participate in the preceptorship of mid-level practitioners, medical students, physician assistant students, and dental residents. Participation in Network Health Fairs, community events, and site/provider meetings is expected, along with meeting all Network and credentialing expectations for licensure and employment. Adherence to the Attendance and Absenteeism Policy is required, recognizing that regular attendance is an essential function. The ability to work at any FHCN location based on business needs is also a requirement. Other duties as assigned.

Requirements

  • Completion of a Doctoral-level degree program with a recognized major with a minimum cumulative GPA of 2.5; or Successful attainment and maintenance of a valid, active professional license required for the role, issued by the appropriate governing or regulatory body.
  • Minimum credit score of 650.
  • Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
  • Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.
  • Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions and other elements.
  • Licensed in the state of California as a Family Medicine Physician.
  • Effective communication of professional opinions and extrapolations of information collected and synthesized/analyzed.
  • Ability to determine appropriate methods of communicating information through the use of tables, graphs, charts and other visual forms.
  • Ability to prepare and execute presentations to large groups.
  • Must have a Doctor of Medicine or Doctor of Osteopathic Medicine degree.
  • Extensive formal training in Family Medicine.

Responsibilities

  • Direct provision of primary care medical services meeting all established productivity expectations.

  • Provide comprehensive health care including health promotion and maintenance, prevention of illness and disability and application of holistic healthcare model with focus on addressing both protective factors and risk factors affecting the health and wellbeing of patient.
  • Manage chronic diseases and acute illness with broad range knowledge in procedural skills, coordination of care with specialists, and utilization of community resources when appropriate.
  • Advocate access to health care with focus of structured evaluation, early intervention and health promotion.
  • Provide excellent customer service and care.
  • Consistently behave with courtesy when interacting with patients/ family members, support staff and Network staff.
  • Remain aware of wait time during the daily workflow.
  • Provide the patient with educational materials and resources appropriate to the patient’s health literacy needs.
  • Achieve patient status as measured by relevant clinical quality measures.
  • Promote and participate in daily huddles.
  • Incorporate the participation of ancillary health team members in managing the care of patients / families.
  • Set and monitor self- management plans.
  • Assist in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.
  • Complete chart and peer review responsibility in a timely manner.
  • Assist the patient care team with performance improvement efforts, staff education and FHCN promotion.
  • Assist the patient care team with performance evaluations and quality improvement.
  • Assist in the preceptorship of mid-level practitioners.
  • Participate in rotation of medical, physician assistant students and dental residents where relevant.
  • Participate in Network Health Fairs & other community events.
  • Participate in site & provider meetings in a meaningful way.
  • Meet Network and credentialing expectations for licensure, and employment.
