Outpatient primary care practice embedded in an Internal Medicine residency program. Join an established team of 2 independent APPs. RN triage team in place. Primarily Medicaid and Medicare. Laboratory services available. Long-standing clinic serving as a healthcare anchor in the community. Meaningful, high-value care for patients with significant social determinants of health. Deliver comprehensive, patient-centered primary care, including high-quality diagnostic and treatment services, to improve the health and well-being of the community. Coordinate seamless care transitions for patients, managing specialized consultations, inpatient admissions, and collaborating with case management for successful discharge planning. Lead or actively participate in continuous quality improvement initiatives to enhance the efficiency and cost-effectiveness of care delivery within the clinic and the wider system. Maintain complete and timely patient documentation and electronic medical records to ensure compliance, accurate records, and support the financial integrity of the practice.
