About The Position

Atrium Health is seeking experienced Nurse Practitioners and Physician Assistants for their Community Care Primary Care Museum Family Medicine location. This full-time opportunity involves caring for underserved, medically complex patients in a diverse practice setting. The role offers embedded care team support, including RN Care Managers, MSWs, CHWs, a Diabetes Educator, and a PharmD. Lab, pharmacy, and interpreter services are available on-site. The location is conveniently situated near Atrium Health Carolinas Medical Center, with access to system-level care management resources and virtual technology.

Requirements

• Experienced Nurse Practitioner or Physician Assistant.
• Primary Care experience is preferred.
• Passion for caring for underserved, medically complex patients.
• Commitment to practicing in a diverse environment that embodies diversity of race, culture, ethnicity, and language.

Responsibilities

• Provide primary care to patients within the Museum Family Medicine location.
• Care for underserved and medically complex patients.
• Collaborate with an embedded care team, including RN Care Managers, MSWs, CHWs, Diabetes Educator, and PharmD.
• Utilize system-level care management resources and virtual technology.
